Thermal vs. Non-Thermal Vein Treatment: Which Is Better?
Vein closure procedures fall into two broad camps: thermal (heat-based) and non-thermal. Each closes the diseased vein effectively, but they differ in anesthesia, comfort, and what's involved.
Thermal ablation
Radiofrequency (RFA) and endovenous laser (EVLA) deliver heat inside the vein to seal it. They have long track records and excellent closure rates. The trade-off: heat requires tumescent local anesthesia (multiple needle sticks around the vein) and carries a small risk of nerve irritation, bruising, or skin burns.
Non-thermal options
Non-thermal methods avoid heat, so they typically skip tumescent anesthesia and reduce nerve-injury risk. They include medical adhesive (which leaves a permanent implant), and foam sclerotherapy (where the agent can sometimes drift beyond the target). Each removes one downside but introduces another.
The trade-off table
In short: thermal means heat and anesthesia but a strong track record; adhesive means no heat but a permanent foreign body; foam means no implant but possible migration. The ideal would close the vein with no heat, no implant, and no drifting agent.
Where the field is heading
SOLVEIN is developing an investigational nonthermal, nontumescent approach that uses a single balloon with integrated coils. The device is withdrawn after treatment, leaving no permanent implant.
